This document sets forth the policies for a reservation made with Lakeside Hideaway Cottage B&B and Lakeside Management. By signing this document I/We acknowledge that I/We have been informed of the policies of Lakeside Hideaway Cottage B&B and Lakeside Management and agree to adhere to said policies. I (we) also acknowledge that the credit card number given shall be charged with any of the below said fees and/or for any damage to the property and/or furnishings that are owed to Lakeside Hideaway Cottage B&B and Lakeside Management. I/We also acknowledge the rates are $150.00 Per Night on weekdays or $200.00 Per Night on weekends/holidays for Double Occupancy OR $200.00 Per Night on weekdays or $250.00 Per Night on weekends/holidays for Quad Occupancy. 6% State Room Tax will be added to these rates.

CANCELLATION POLICY:

A $50.00 fee shall be charged if you cancel once reservation has been made. If you cancel less than 14 days prior to your arrival date, then the Full Amount of your reservation will be charged. If you are a No Show or if you leave early, you will also be charged the Full Amount of your reservation. 72 Hour Notice of Cancellation Must be given on all scheduled Massages or full amount will be charged. ALL Cancellations must be made by Phone (person to person)--NO voice mail or E-mails or faxes will be accepted for cancellations. **A cancellation number Will be given for verification when cancellations are made.**

POLICIES:

No Smoking (Outside Only)

No Children

No Pets

No more than 4 guests on property at anytime

No Visitors of Guests

No Loud Music or Parties

CHECK-IN Time is anytime after 3:00pm

CHECK-OUT Time is by 11:00am. (We are happy to accommodate early check-ins or late check-outs when we are able. There shall be a charge of $25 per half-hour that you occupy the house past 11:00am or before 3:00pm if you do not request and we approve a late check-out/early check-in).

Please be kind and treat the cottage as if it were your own. The cottage and grounds must be left in the same condition as it were upon check-in. If anything is damaged at the cottage caused by the misuse of guest(s) or stolen, the guest(s) shall be responsible for the full amount of damage or value of item(s) stolen and this amount will be charged to the credit card number given below. If cleaning beyond the “normal” range is required, a charge of $75.00 shall be charged. If you choose to use the kitchen for cooking, please be kind and leave it the way you found it--if kitchen is not left clean, a charge of $25.00 shall be charged. A $25.00 fee shall be charged for lost or unreturned keys. A $50.00 fee shall be charged if the Hot Tub requires cleaning beyond the "normal" range and/or if the cover has not been replaced after each use. Lakeside Management and/or their contracted service person(s) reserves the right to enter the house to make needed repairs and/or to inspect the property if management feels that any of the above mentioned policies are not adhered to by guest(s).

I/We agree and understand that at any time there are more than the reserved number of guests in the cottage or on the property, any children, inside smoking, any pets, loud music, parties, unacceptable behavior or noise, or any illegal activity, the guest(s) shall be required to leave the premises of Lakeside Hideaway Cottage B&B and the full amount of reservation will be forfeited by guest(s).

I/We agree and understand that scheduled Massage have a 72 Hour Cancellation Policy. If a 72 Hour Notice of Cancellation (at least 72 hours Prior to Scheduled Start Time of service) is not given, then the Full amount due for scheduled Massage(s) will be charged to the credit card number given below.

I/We agree to indemnify and hold harmless the owner of the cottage, Lakeside Management, and their agents from any accidents or injury suffered by I/We on or about the cottage or premises Or for any loss of money, jewelry, or valuables of any kind. Use of hot tub shall be at guest(s) own risk. Guest(s) with high blood pressure, any heart conditions, or if they are pregnant should Not use the Hot Tub. I/We will inform the Managers immediately of any unsafe or hazardous condition or needed repairs in or about the house or premises. Guest(s) will inform the Managers immediately of any unsafe or hazardous condition or needed repairs in or about the house or premises. No refunds or discounts will be given for Hot Tub, A/C, Heating, Appliance(s), Electronic Equipment, Water and/or Sewer failures. Needed repairs will be made as soon as possible. No refunds or discounts will be given due to weather conditions and/or conditions of the lake.

By signing below, I/We acknowledge that I/We have received and reviewed the Reservation Form and agree to all said policies.

**Credit Card Number is verified when reservation is made for security damage deposit. 1/2 the Total Amount amount of Reservation is due with this form (either by Credit Card, Check, or Money Order--Made Payable to Lakeside Management. Valid Credit Card # and Signature Required below even if paying by Check or Money Order for Security Deposit. Final Amount Due will be due 14 days prior to your arrival date.**
